Job description
Advanced Clinical Practitioner in Frailty

Salisbury Hospital is expanding its Frailty Same Day Emergency Care (SDEC) service and we are looking for enthusiastic Advanced Clinical Practitioners (ACPs) to join our dedicated team. This is an exciting opportunity for clinicians who thrive in fast‑paced, dynamic environments. You will work alongside Consultants, Therapists and Community Partners to deliver Comprehensive Geriatric Assessments within the Emergency Department and SDEC areas.

Main duties
  • Provide advanced clinical practice and deliver Comprehensive Geriatric Assessments for older patients in ED and SDEC.
  • Utilise your ACP skill set to assess, examine, diagnose and manage patients with undifferentiated presentations.
  • Support same‑day discharge and admission avoidance, working closely with community partners including Hospital@Home virtual wards.
  • Develop expertise in managing common conditions affecting older people (Heart Failure, Falls, Parkinson’s Disease).
  • Contribute to service development as SDEC expands across the Trust.
  • Supervise, support and teach ACP trainees and colleagues within the multidisciplinary team.
Qualifications – Essential
  • Previous NHS experience.
  • Registered General Nurse or Registered Allied Health Professional with current NMC or HCPC registration.
  • Master's Degree in Advanced Clinical Practice covering all four pillars of ACP capabilities.
  • Registered Non‑Medical Prescriber (where applicable). Recent experience managing Acute Frailty presentations in an acute hospital setting.
  • Significant post‑registration experience at band 7 working with older adults.
  • Evidence of proactive involvement in supervision and development of team members and change‑management skills.
  • Proficient in computer systems, spreadsheets, databases, and report writing.
  • Understanding of current NHS developments in Frailty and Older People.
  • Ability to evaluate care to improve quality standards and service improvement.
Qualifications – Desirable
  • Teaching and supervision/mentorship qualifications.
  • Leadership or management qualification.
  • IRMER training.
  • Evidence of research involvement.
  • Ability to prescribe mobility aids and demonstrate interdisciplinary skills beyond traditional boundaries.
