Job summary
A healthcare provider in Chicago seeks a dedicated individual to oversee daily operations of Teams of Workers (TOWs) and provide patient outreach. The role requires a high school diploma, at least 5 years of experience in social services, and supervisory skills. Ideal candidates will excel in communication, teamwork, and adaptability. Responsibilities
- Monitors TOWs to help the team meet their individual and team goals.
- Creates a healthy and motivating work environment.
- Creates workflows/procedures with external stakeholders.
- Oversees day-to-day operations of TOWs.
- Provides patient outreach and resource navigation.
- Engages in community outreach and advocacy.
