We are looking for an enthusiastic and adaptable Team Lead to join our caring and progressive team. As an organisation we support the NHS graduate guarantee and this would be an ideal opportunity for a newly qualified nurse.

Our hospitals are nurse-led and focus on holistic rehabilitation and therapy for people who need extra help to get home, or who are nearing the end of their life.

It's a relaxed atmosphere, but it's never a breeze - you'll need to think on your feet and be confident in making independent clinical decisions on behalf of the patients in your care.

Main duties of the job

We're looking for experienced and dedicated nurses with relevant clinical skills and who is ready to embrace the unique challenges and rewards of community nursing.

Candidates wanting to move to a community setting with advanced clinical skills or expertise will be considered. KCHFT is keen to support development and therefore, candidates without the specialist District Nurse qualification or having a recognised advanced clinical skills/module will be considered for trainee posts.

Using the named nurse model, we want to allow nurses to spend more time with patients, promoting self-management where possible, and improving patient lives.

You’ll need to have a full UK driving licence and drive your own car, but don’t worry – you can claim mileage costs and access the NHS car lease provision after your probationary period.

Person Specification
Qualifications, training and experience
  • You will have a Level 6 Degree or equivalent qualification, or equivalent knowledge acquired through experience plus a diploma in nursing
  • You'll need to have experience of managing complex caseloads and supporting/ mentoring others
  • You will be a registered health care professional with the Nursing and Midwifery Council.
Knowledge and skills
  • You’ll need have sound communication skills in order to understand and communicate both verbally and in writing
  • You will need to have a customer focussed approach which is essential to; o Provide patients, clients and their families with the best possible experience o represent the Trust when talking to and working with patients, colleagues and visitors. o Work together with colleagues and other teams across the Trust.
  • You will need to be able to use IT systems and databases
  • Having an awareness of developments within the NHS would be an advantage as well as an understanding of National Service Frameworks and NICE guidance.
  • You will have the ability to focus on the details and spot any potential problems
  • You will have knowledge of up to date clinical practice by using up- to- date skills and adhering to evidence-based procedures and clinical effectiveness frameworks. You will fully understand the role and remit of a health care worker.
Working environment
