A specialist supported living provider in England seeks Support Workers to help individuals with learning disabilities and mental health needs. Our specialist Supported Living and Outreach service in St Cleer provides support for people with Learning Disabilities, Autism, Mental Health needs and Acquired Brain Injuries
We work in partnership with Local Health Professionals such as social workers, community nurses, and specialists such as Speech and Language Therapists, Physiotherapists and Occupational Therapists. Person Centred Planning Our staff support each person to create a person-centred plan that tells us how they want to live their life now and in the future. Through a range of communication tools, we ensure that each person is closely involved in the development of their own plan.
As a Support Worker, you will be involved in:
