We’re excited to offer a locum opportunity for an experienced Social Worker to join the Adult Discharge to Assess service in Bristol. This full-time, ongoing role pays £32 per hour and plays a crucial part in supporting adults as they transition safely from hospital care to the community, empowering them to regain independence with dignity and support.

What You Will Do
  • Conduct timely assessments for adults being discharged from hospital settings.
  • Create and implement person-centred care plans tailored to individuals’ unique needs and goals.
  • Work closely with healthcare teams, community services, and families to coordinate safe, effective discharges.
  • Monitor progress, reviewing and adjusting care plans to support long-term well-being.
  • Offer guidance and support to individuals and carers, helping them make informed care decisions.
  • Maintain accurate, compliant documentation aligned with statutory frameworks and best practice.
  • Advocate for service users’ rights, choices, and dignity throughout the discharge process.
