Social Care Assessor – Cheshire East Council

Cheshire East Council is seeking a dedicated and compassionate Social Care Assessor to join a supportive and forward‑thinking team working with vulnerable adults and their carers across the borough.

This is a fantastic opportunity for an experienced social care professional who is passionate about person‑centred practice, multi‑agency working, and making a meaningful difference to people’s lives.

The Role

As a Social Care Assessor, you will work within relevant legislation, council policies, and procedures to assess needs, manage care plans, and coordinate effective support packages for vulnerable people and their carers.

You will manage a varied caseload, balancing assessment, care planning, review, and partnership working to ensure individuals receive the right support at the right time.

Key Responsibilities
  • Undertake needs‑based assessments with vulnerable people and their carers, identifying risks, priorities, and appropriate responses
  • Develop, implement, and maintain individual Care Plans, acting as Care Manager
  • Monitor, review, and adapt Care Plans in response to changing needs
  • Liaise and negotiate with health professionals, statutory bodies, voluntary organisations, carers, and families
  • Complete financial assessments, explain care costs, and support access to benefits advice
  • Maintain accurate and up‑to‑date electronic and written records
  • Work flexibly across Cheshire East in line with service needs
About You
  • Confident, organised, and empathetic professional with experience supporting vulnerable people in social care or health settings
  • NVQ Level 3 (or equivalent) in Social Care
  • Experience working with vulnerable people and their carers
  • Strong understanding of needs‑based assessments, care management, safeguarding, and risk assessment
  • Ability to work effectively within multi‑disciplinary teams
  • Good IT skills and confidence using electronic systems
  • Excellent communication, negotiation, and problem‑solving skills
  • Ability and willingness to travel across the borough
  • NVQ Level 4 (or equivalent) in Social Care
  • Experience working across multiple social care settings
  • Knowledge of local services and wider community resources
