Job summary

A leading hospice organization in Oxford is seeking a Patient Care Manager to join their leadership team. In this role, you will ensure the delivery of compliant and efficient patient care, while providing support and education to team members. The ideal candidate will have a background in nursing with hospice experience, strong communication skills, and be sensitive to the needs of terminally ill patients. Qualifications

  • Minimum of three (3) years of hospice experience required.
  • Minimum one year of field-based nursing experience is recommended.
  • Valid license as a Registered Nurse in the state of employment.

Responsibilities

  • Responsible for team compliance with policies and procedures.
  • Assists with adherence to policy and procedure manuals for quality care.
  • Ensures compliance with regulatory requirements including CMS submissions.

Education

Graduate of an accredited school of nursing
Certification as a Certified Hospice and Palliative Care Nurse (CHPN)
