Job Summary
Palliative Care can be offered to patients when their illness can no longer be cured. This is not only for people with cancer but also other illnesses like heart failure and respiratory diseases.
Palliative Care shifts the focus from “getting better” to “feeling well” whilst managing an incurable illness; optimising quality of life and then supporting patients and their families as they approach the end of their lives and are dying, focusing on dignity and comfort.
Main duties of the job
- High quality palliative care support and interventions across the Acute Trust and Community.
- Support the delivery of training to specialist and non‑specialist colleagues across the Trust to support best practice in palliative and end of life care.
- Work collaboratively with generalist colleagues to support excellent care both directly and indirectly using influencing, situated learning and role‑modelling.
- Work within your scope of practice, seeking advice from senior expert colleagues in complex patient care and delivering the plan of delegated care to the highest standard.
- Opportunity for an enthusiastic and experienced staff nurse to develop into a more specialist field, while delivering high quality safe and effective palliative care support and guidance.
Responsibilities
- Assess and reassess the needs of patients with specialist palliative care needs under the guidance of the senior Specialist Palliative Care nurse on duty. Within own scope of practice and the palliative care framework, plan, implement and evaluate individualised holistic nursing care.
- Provide effective evidence‑based nursing care to the highest standard delegated by the senior Specialist Palliative Care Nurse on duty.
- Be confident and competent in all areas of palliative nursing practice, understanding own limitations.
- Participate in audit for clinical practice and assist with data collection for research.
- Exercise compassionate counselling skills and act in an advisory and guiding role in all aspects of nursing care for both patients/clients and members of the team.
- Play an active role in best practice, ensuring hygiene and cleanliness, disseminating good practice; participating in audits; benchmarking; raising awareness of service users; challenging colleagues.
- Ensure accurate and clear records are kept according to NMC Guidelines for Records and Record Keeping.
- Comply with the NMC codes of practice and understand scope of practice when working as a staff nurse within a specialist team.
