A medical care organization in Rochester, England, is seeking a compassionate Registered Nurse to provide ongoing medical care to long-term residents. The role involves managing various clinical needs, working within an interdisciplinary team, and documenting patient care. Candidates must hold an MS in Nursing as well as RN and NP certifications from New York State. The pay rate is $75.00 per hour, reflecting the expertise required in geriatric care.
Provide continuing medical care to long-term care residents with multiple medical and surgical diagnoses and to residents with restorative diagnoses on a short-term basis.

Clinical care. Evaluation and management of acute, sub-acute and chronic clinical needs of older adults. Collaborate with primary geriatrician on functional changes, patient-specific goals and appropriate blend of curative and symptom focused treatment plans
Care Management. Complete comprehensive clinical reviews of all chronic conditions and document in medication record. Assess and reassess patient specific goals for current and future care. Review and evaluate specialty recommendations in conjunction with patient specific goals and preferences.
Interdisciplinary team Administrative Duties. Attends morning meeting and team care planning meetings, in collaboration with other primary care staff. Reviews lab results & specialty notes, initiating orders where appropriate. Complete pre-enrollment visits to orient new patients to ElderONE medical practice and PACE model of care. Coordinate medical discharge planning documentation, collaborating with IDT members.
Infection Control. Utilize infection control equipment and safety guidelines to minimize and/or prevent occupational exposure to communicable diseases, potentially infectious body fluids and risks of prescribed patient treatment (i.e., radiation, toxic substances).
M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.
