• Energetic social worker with a strong commitment to person-centred practice.
• Registered with Social Work England and able to pass an Enhanced DBS check.

Responsibilities

• Conduct strengths-based assessments and support planning.
• Perform Mental Capacity Assessments and participate in Mental Health Act engagements.
• Collaborate with multidisciplinary colleagues.
• Complete S117 assessments and reviews.
• Identify carers’ needs and provide assessment support.
• Contribute to service development within the community mental health service.
