Job summary

A community health organization is seeking a Family Health Worker to join their 0-19 Service in Leeds. This role involves assisting in the Healthy Child Programme, conducting health needs assessments, and providing support to children and families using a partnership approach. If you have a passion for influencing outcomes for children and families, come and join our innovative and forward-thinking 0-19 Service as a Family Health Worker (various hours available).

Our staff led the way moving from the title of Community Nursery Nurse to this role to reflect the changing dynamic of our work with families across the city. Your role will be to assist in the delivery of the Healthy Child Programme, offering support and interventions to children and their families. You will be actively involved in seeking out health needs through detailed assessments, therefore you will need to have the skills and knowledge to identify and respond to these. Awareness of safeguarding issues is crucial alongside the ability to identify and appropriately respond to concerns. We work using a partnership approach with the multi-agency team and wider agencies therefore good verbal and non-verbal skills are essential in ensuring local health needs and priorities are addressed.

Alongside 0-19 colleagues and partner agencies you will demonstrate effective team working and communication skills in supporting families through timely interventions, to understand key developmental milestones, promote healthy lifestyles and school readiness. A typical working week could include: completing development reviews in clinics, Children’s Centres and the family home; facilitating breastfeeding groups and individual one to one feeding support; offering face to face and virtual support to families via virtual platforms and in our integrated Baby Hubs across the city; working with our key partners to deliver the HENRY programme; supporting parents through our evidence-based pathways with positive parenting interventions such as toileting, nutrition, managing child behaviour/ sleep, speech and language and overall child development; signposting to other key professionals and completing referrals as appropriate.
