Hospital at Home Staff Nurse - Band 5

£34,521 - £41,956 Inclusive of HCAS per annum pro rota

Full time - 37.5 hours per week (8-8pm)

Bromley Hospital at Home is a new integrated service delivered through Bromley Healthcare and One Bromley partners for avoiding acute hospital attendances and admissions while also supporting patients’ early discharge from acute hospital beds.

The service provides diagnosis, treatment and monitoring of patients in their usual place of residence using both face to face and virtual care in the London Borough of Bromley. Operating 7 days per week 8am to 8pm. Referrals are accepted to the service, from the Acute Trust - Princess Royal University Hospital (PRUH) & Bromley Healthcare Urgent Community Response - Rapid Response Team.

One Bromley, the place-based partnership of health, care and third sector organisations in Bromley has significant ambitions for the service.

Joining this service now provides the unique opportunity to help shape and develop the service both as it starts to operate at scale and as it continuously develops over the coming years.

Main duties of the job
  • To comply with the duties placed on employees by the Bromley Healthcare’s Health and Safety Policy and Related Procedures and to act in accordance with all instruction, information and training supplied in relation to those duties.
  • Such other duties as requested, compatible with the post, which may arise from time to time and flexibility to support the services within UCR.
  • Observe confidentiality of records and health information at all times and be aware of and practice the principles of the Data Protection Act of 1984, as this applies to computerised information.
  • To undertake appropriate training to meet individual developmental needs within the role.
  • Undertake the measurement and evaluation of work and current practices as directed by team lead as part of the clinical audit programme.

To participate in achievement of service objectives to demonstrate productivity, performance and effectiveness

Detailed job description and main responsibilities
  • To undertake a focussed person-centred approach, ensuring quality standard and delivery of care for patients.
  • To develop safe clinical skills in community setting to proactively manage very high intensity users of health and social care services, with multiple co-morbidities and pharmacy, social, medical and nursing needs and who are at risk of hospital admission.
  • To undertake nursing procedure as indicated for example, catheterisation, monitoring of vital signs and review of symptoms, phlebotomy, IV therapy and cannulation in the patient’s home.
  • To proactively monitor and manage a patient caseload with sound clinical autonomous decisions about patient care, and seek support from colleagues as appropriate.
  • To confer, as appropriate and work with the patient's GP, BHC GP/Consultant, Head of Hospital at Home, Operational Manager, ANP, ACP and/or Out of Hours GPs to ensure there is robust communication around clinical issues as indicated.
  • To sensitively deal with distressing or emotional circumstances regarding patient care, and communicate information, sensitively, requiring empathy and reassurance, to patients and carers.
  • To actively seeking GP/ Consultant to review polypharmacy and other medication issues in accordance with the Department of Health, Professional and organisational guidelines.
  • To work effectively as part of a multidisciplinary team working to support the common goal of safe, efficient care delivery closer to the patients’ homes.
  • To empower patients and their families through appropriate support and education.
  • To utilise formal and informal reporting mechanisms, including electronic and handwritten reporting, to ensure effective internal and external communications and to ensure all documentation is accurate and professional.
  • Fully support the use of EMIS mobile and provide contemporaneous clinical records.
  • To continuously challenge practice, ensuring a sound evidence base, thus meeting clinical governance requirements.
  • Promote multidisciplinary team working to provide rapid crisis intervention, with the common goal of safe, efficient care to people in their own homes.
  • Support the Operational Manager in delivering the UCR KPIs and ensure that capacity is maintained, and timelines are managed at times of escalation.

To contribute to partnership working with Acute Trust GP’s, Nursing/Residential home staff and other health care professionals within Bromley Healthcare, Social Services and voluntary agencies to promote an anticipatory care planning

Qualifications
  • Diploma/Degree Adult Nursing

Experience
  • Actively involved in clinical supervision
  • Experience of working in/with a multi-disciplinary team
  • Acute or relevant nursing experience
  • Knowledge of Hospital at home and Virtual wards
  • Working in an Intermediate Care setting.
Skills
  • To have organisational skills such as delegation, prioritisation and negotiation.
  • Sound knowledge and understanding of Clinical Governance.
  • Car driver with clean current UK licence and access to a vehicle to use for work
  • Competent to administer intravenous drugs
  • Cannulation and phlebotomy skills.
  • Qualified & competent to carry out male/female including suprapubic catheterisation
  • Knowledge of the Management of patients with Heart failure with fluid overload
  • Experience of clinical audit.
  • Report writing skills
  • Understanding of Research & Development.
  • An understanding of Change Management.
