Qualifications

Responsibilities

• Manage a caseload of adults in hospital requiring assessment and discharge arrangements.
• Undertake Care Act assessments and support planning.
• Work with hospital staff and community services for discharge facilitation.
• Prepare high-quality professional reports for safeguarding meetings and Court of Protection.

Skills

Knowledge of Care Act 2014

Experience in Adult Social Care
