As Head of System Flow & Care Integration at the Royal Borough of Windsor & Maidenhead, you will provide strategic leadership and professional oversight to ensure safe, timely, strengths‑based pathways that support effective hospital discharge, prevent unnecessary admissions and maximise independence.
You will lead the transformation of intermediate care, working across health, housing, social care, and the voluntary and community sector to develop integrated, person‑centred pathways that promote prevention, reablement, and community‑based support.
This role contributes directly to Living Well Together—RBWM’s transformation vision—supporting the development of inclusive, healthy communities, improving system flow, and embedding high‑quality, compassionate, and dignified care for residents.
