Qualifications

• Minimum 1 year experience in Geriatrics, Frailty, General Medicine, or Community Nursing.
• Strong understanding of elderly care pathways.
• Confident working both independently and as part of an MDT.

Responsibilities

• Supporting consultant-led geriatric & frailty clinics.
• Completing comprehensive patient assessments.
• Managing patients with common geriatric conditions.
• Accurate record-keeping within NHS electronic systems.
