Overview

We are seeking to recruit a newly qualified or experienced Band 5 community nurse to join our team. We want you to be a person who values development, be self led and confident. Our care community team is based in Ellesmere Port and covers the local area. It’s important that you have access to your transport for work purposes, mileage expenses are reimbursed in addition to your hourly rate.

Cheshire and Wirral Partnership (CWP) provides health and care services for a population of over one million people, including mental health, learning disability, community physical health and all-age disability care, as well as the provision of three GP surgeries in Cheshire. We employ around 4,500 staff across 73 sites and have services across Wirral and Cheshire, as well as Trafford, Warrington, Bolton, Halton and Liverpool.   • You will be proactive in coordinating their care needs as appropriate, avoiding unnecessary admission to hospital or long-term care settings.
