Graceful Healthcare Services is looking for dedicated Senior Support Workers to join our compassionate team who daily support citizens with complex care needs such as Autism, Learning and Physical Disabilities etc.

About the Role

As a Senior Support Worker, you will provide person-centred care to individuals in their own homes, supporting people who live with a variety of health needs, such as but not limited to Learning disabilities, Mental health conditions, Physical disabilities, Complex care support, dementia care, and more. You will be involved in care planning and review, activity management, and planning. You will also be involved in the daily audit of food and groceries to ensure that the service users are living healthy, while nutrition is promoted.

You will play a vital role in ensuring that individuals in your care live fulfilling, independent, and happy lives and experience an improved quality of life (QoL).

Responsibilities

Your responsibilities will include:

  • Assisting with personal care and hygiene routines
  • Administering and managing medication in strict compliance with care plan and MAR Chart
  • Daily environmental risk assessment and risk management of the supported placement property
  • Supporting individuals to access the community, engage in social activities, and develop healthy relationships.
  • Encouraging independence in daily tasks such as meal preparation, household chores, and financial management
  • Maintaining a clean, safe, and accessible home environment
  • Supporting with day-to-day activities, including attending appointments, shopping, and engaging in hobbies
  • Assisting individuals in managing emotions and behaviours, promoting positive behaviour.
  • Providing companionship and emotional support
  • Supporting individuals in external settings such as hospitals, holidays, or police stations if required
  • Keeping accurate records and reporting to the Care Placement Manager
Working Hours

We offer flexible shifts to suit your lifestyle, including:

  • Days or nights available – full days (12 hours).
  • Sleep-in and waking night shifts are also available.
  • We may be able to offer a tailored shift which will be based on a need‑by‑need basis. However, this must be discussed at the interview stage.

Training and Certifications

Due to the role involving supporting individuals with complex care needs, here are some of the qualifications/trainings that will help you succeed in the role;

  • NVQ Level 3 in Health and Social Care
  • PMVA/MAPA/Any relevant qualifications attained in de‑escalation techniques
  • PBS training/certification
