Shift patterns

Shifts may include days, nights, evenings, or weekends.

Key Duties And Responsibilities
  • Undertake daily living tasks as agreed with the service user, their family, and relevant professionals.
  • Work within the policies and procedures of Connected Living.
  • Support care and risk assessments specific to each service user’s needs.
  • Complete Health and Safety checks in line with established policy.
  • Work confidently as a lone worker in line with lone‑worker risk assessment.
  • Ensure medication is delivered and ordered safely to the service user’s home.
  • Provide opportunities for service users to express their preferences on task delivery.
  • Maintain and promote choice and independence for service users.
  • Manage service users’ behaviours in the least restrictive way possible.
  • Complete daily notes, handovers, and relevant documentation by the end of each shift, ensuring high‑quality handovers.
  • Work as part of a team and wider multidisciplinary team (MDT) to support clients’ emotional and physical wellbeing.
  • Develop and maintain professional working relationships with service users and MDT to ensure positive outcomes.
