A community health service provider is seeking enthusiastic Staff Nurses to join their Neighbourhood Team in Yeadon, England. We are based in Yeadon Health Centre, but we cover Rawdon, Yeadon, Guiseley, Menton, Pool in Wharfedale, Otley and the surrounding area.
Our community nursing teams deliver planned care in patients’ homes, helping people to remain independent and supported within their local communities. We provide a wide range of clinical interventions, including (but not limited to):

The service runs 8am to 8pm, 7 days a week, with various shift patterns.
As a Staff Nurse in our Neighbourhood Teams, you’ll be part of a compassionate and highly skilled multidisciplinary team, working alongside District Nurses, Community Matrons, Occupational Therapists, Physiotherapists, Neighbourhood Clinical Assistants, Therapy Assistant Practitioners, and Self‑Management Facilitators to deliver truly holistic, person‑centred care.
You’ll also collaborate closely with our Citywide Services, including the Falls, Continence, Urology and Colorectal Care, and Palliative and End of Life Care teams, as well as GP services, ensuring the best possible coordination and continuity of care for patients across West Leeds.
We’re looking for proactive nurses who are motivated, adaptable and caring, with a passion for community healthcare. You’ll bring strong communication skills, a commitment to delivering excellent clinical standards, and the resilience to work in a dynamic and ever‑changing environment.
