This service provides high-quality, person-centred support to adults with learning disabilities and their families, enabling independence and inclusion in the community.The successful candidate will support individuals who have short stays for respite within our Short Breaks service. You will play a key role in promoting choice, dignity, and support individuals to access community facilities while promoting healthy sleep patterns to enhance well being.Key Responsibilities will be to deliver flexible, responsive, and enabling care that promotes independence. Supporting the individual in daily living tasks and community engagement. Contributing to support planning and risk assessment processes, whilstworking collaboratively with the Senior Social Care Worker and wider team.You should have excellent communication skills and a person-centered approach. Demonstrate professionalism and a commitment to social inclusion.
