You will work both autonomously and as part of a supportive team to deliver high‑quality, person‑centred care for in‑patients and outpatients in our growing Wellbeing service. You will lead a Wellbeing exercise programme and guide patients through their personalised exercise plan.

Key responsibilities
  • Provide clinical and administrative support to the Rehabilitation Team in delivering palliative rehabilitation.
  • Carry out moving and handling assessments, including prescribing and ongoing evaluation of equipment.
  • Maintain accurate patient records in accordance with policies.
  • Empower patients through self‑management strategies and education.
  • Collaborate with equipment suppliers, GPs, community nurses, physiotherapists, and other healthcare professionals to arrange smooth and safe discharges when needed, and order appropriate equipment.
  • Support service development and participate in group sessions and staff training.
  • Deliver breathlessness, fatigue, and anxiety management classes and sessions.
  • Assess and provide mobility aids; ensure patients receive relevant information and support.

Person specification
Experience
  • Experience of working in a Health Care Setting
  • Experience of Multidisciplinary Team working
  • Experience of working directly with patients providing care or treatment interventions
  • Knowledge of anatomy and physiology related to clinical area Knowledge of Moving and Handling guidelines
  • Evidence of working at Band 4 level within the NHS
  • Previous experience working with Oncology/Palliative Care patients Experience supporting with administrative tasks
Qualifications
  • 5 GCSE level passes at C or above, including English and mathematics (or equivalent) Or NVQ Level 3 Health and Social Care (or equivalent)
