A community-focused healthcare provider in Leeds seeks a dedicated Registered Nurse to join the Neighbourhood Response Team. An exciting opportunity has arisen for enthusiastic and motivated Staff Nurses to join our developing Neighbourhood Response Team at Leeds Community Healthcare. This is a fantastic time to become part of a Response Service that is growing, evolving, and shaping new ways of delivering urgent community care across the city. The Neighbourhood Response Team is a core component of the NHS community services commitment to the NHS 10-year plan and development of Neighbourhood Health.
Our Neighbourhood Response Team provides timely, person-centred nursing care within patients’ homes, supporting early discharge from hospital and preventing unnecessary hospital admissions. We work to clear timeframes of 0–2 hours, 2–4 hours and 4-48 hours, ensuring patients receive the right care, at the right time, in the right place.
We have bases across Leeds and offer a variety of flexible shift patterns between 7am and 6pm, seven days a week.
