Practice Nurse - The Avenue Surgery The Avenue Surgery, Brighton, BN2 4GF

Sussex Primary Care (SPC) is a GP Federation presenting a unique opportunity to join a supportive, learning organisation dedicated to delivering quality health care to over 50,000 patients. We are seeking a Practice Nurse to provide high‑quality nursing care across acute, chronic, preventive and promotional services.

Job Summary

The post holder is responsible for the delivery of general practice nursing care to the whole practice population, ensuring evidence‑based practice for acute and long‑term conditions and providing preventive health care for all patients.

Job Responsibilities
  • Patient Care
    • Assist and perform routine tasks: wound care, Doppler, suture and clip removal, urinalysis, chronic disease management, venepuncture, immunisations, childhood immunisations.
    • Provide specific clinical services including: cytology, travel clinic, asthma & COPD management, hypertension (including 24‑hour ambulatory BP), diabetes care, ear syringing, chaperoning.
    • Order and monitor pathology tests; manage vaccine fridge and interpret temperature logs.
    • Maintain clinical rooms, equipment and stock; order vaccinations and health promotion literature.
    • Record accurate patient information and contribute to administrative systems.
    • Attend and participate in practice and clinical meetings.
    • Carry out home visits when required; support QoF programmes and income generation.
    • Support the supervising HCA as appropriate.
  • Communication
    • Use sensitive communication styles and obtain informed consent.
    • Communicate clearly with patients and carers, adapting to cultural, language and comprehension needs.
    • Advocate for patients and colleagues, providing support for difficult news and referrals.
    • Maintain effective communication within the practice and with external stakeholders.
  • Quality & Governance
    • Produce accurate, contemporaneous records and comply with NICE, NSF and NMC guidelines.
    • Conduct peer review, audit and quality improvement activities; participate in shared learning.
    • Apply risk management to protect staff and patients; oversee vaccine and drug safety.
  • Leadership & Team Working
    • Support the lead nurse/ANP; model professional behaviour and promote the role of practice nurse.
    • Work collaboratively; delegate appropriately; develop new ways of working.
    • Lead and manage projects or specific services as agreed with practice management.
  • Information Management
    • Use technology and software to manage care plans, Read codes, and audit data.
    • Ensure compliance with FOI and information‑sharing guidelines.
  • Learning & Equality
    • Mentor junior staff and foster a culture of continuous improvement.
    • Address issues of discrimination, support equality and diversity initiatives.
    • Facilitate patient education and promote access for marginalised groups.
Person Specification
Qualifications
  • Registered Nurse (RN) – Essential.
  • Relevant GP or primary care experience – Desirable.
  • Specialist primary‑care courses (e.g., CHD, diabetes, asthma & COPD, infection control) – Desirable.
Experience
  • Experience as a qualified nurse in a GP setting – Essential.
  • Prioritisation of patient needs and informed decision‑making – Essential.
  • Knowledge of primary‑care policy and national legislation – Essential.
  • Experience in supervising or line management – Desirable.
  • Experience working within a GP practice – Desirable.
