We are looking a registered nurse to join ourexisting team of experienced practice nurses. We are looking for someone withexperience of working in accordance with the UKCC Code of Professional Conductand working within primary care. They must have a demonstrable commitment toprofessional development.
The job role will require you to work Monday 8:30-20:00, Wednesday 8:30-18:30 and Thursday 8:30-18:30.
Ournew Practice Nurse can expect to work in an encouraging, supportive, forwardthinking, experienced Practice environment that is continually working toimprove the quality of healthcare for our patient population.
OurPractice Nurse role would be expected to undertake a range of nursingassessments and provide appropriate care or treatment in conjunction with ourclinical team and Practice Policies.
Ideally,you'd be able to support and deliver a high level of quality care and servicebased on the needs of the patient population of which would include treatment,management plans, screening, preventative care, and patient education. Inaddition to this, you would also be expected to support the Practice in meetingits contractual obligations and delivery of enhanced service specifications.

To be responsible for the implementation ofprocesses for the effective management of patients with long-term conditionsusing evidence-based practice including care for elderly and houseboundpatients.
Working as a senior member of the practice multidisciplinaryteam, the post holder will ensure nursing services are delivered effectively tothe entitled patient population.
The Practice Nurse will be responsible for severalclinical areas such as infection prevention and control, health promotion, chronic disease management, healthpromotion, well women and well man clinics, as well as actively supporting thepractice management team in the reviewing and delivery of clinical policy andprocedure.
