Job description

We are looking a registered nurse to join ourexisting team of experienced practice nurses. We are looking for someone withexperience of working in accordance with the UKCC Code of Professional Conductand working within primary care. They must have a demonstrable commitment toprofessional development.

The job role will require you to work Monday 8:30-20:00, Wednesday 8:30-18:30 and Thursday 8:30-18:30.

Ournew Practice Nurse can expect to work in an encouraging, supportive, forwardthinking, experienced Practice environment that is continually working toimprove the quality of healthcare for our patient population.

OurPractice Nurse role would be expected to undertake a range of nursingassessments and provide appropriate care or treatment in conjunction with ourclinical team and Practice Policies.

Ideally,you'd be able to support and deliver a high level of quality care and servicebased on the needs of the patient population of which would include treatment,management plans, screening, preventative care, and patient education. Inaddition to this, you would also be expected to support the Practice in meetingits contractual obligations and delivery of enhanced service specifications.

Main duties of the job

To be responsible for the implementation ofprocesses for the effective management of patients with long-term conditionsusing evidence-based practice including care for elderly and houseboundpatients.

Working as a senior member of the practice multidisciplinaryteam, the post holder will ensure nursing services are delivered effectively tothe entitled patient population.

The Practice Nurse will be responsible for severalclinical areas such as infection prevention and control, health promotion, chronic disease management, healthpromotion, well women and well man clinics, as well as actively supporting thepractice management team in the reviewing and delivery of clinical policy andprocedure.

We are afriendly, professional, and caring team who are passionate about providing safeand effective care. Our excellent multi-skilled Clinical Team comprises of 22 GPs (Partner & Salaried), Pharmacists, Pharmacy technicians, PracticeNurses & HCA's.

Our teamalso includes a full administration team, including a reception team who workin a Patient Advisor role providing enhanced signposting and ensuring patientshave access to the appropriate clinicians or services available to them.

Job responsibilities

The following are the core responsibilities of the Practice Nurse. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels:

  • Support the Infection Prevention Control (IPC) lead and to work to the IPC Policy supporting the implementation of audits, training, support and evidence of compliance to the practice manager
  • Support the production of the IPC annual statement in conjunction with the practice manager
  • To develop, implement and embed an effective care management programfor the frail and housebound patients registered at the practice
  • To assess the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence-based practice
  • To implement and evaluate individual treatment plans for chronic disease patients that promote health and wellbeing
  • To identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to patients health
  • As required, to provide routine nursing care to patients in accordance with clinical based evidence and NICE guidelines
  • To prescribe medication for patients in accordance with PGDs and own scope of practice
  • To review patient medications to enhance compliance
  • To provide guidance to patients in the use of prescribed and over-the-counter medications regarding side effects and dosages
  • To co-ordinate the provision of travel medicine services
  • To request pathology services and process pathology results as required
  • To oversee the provision of chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required
  • Routine treatment room duties including dressings and wound care, removal of sutures and clips, blood pressure monitoring, venipuncture, ECG, cervical cytology and swabs
  • To maintain accurate clinical records in conjunction with extant legislation
  • To ensure SNOMED codes are used effectively
  • To ensure the appropriate maintenance and use of chronic disease registers
  • To oversee QOF chronic Disease registers, monitor progress and assist the practice in the achievement of targets.
  • To develop, implement, embed and participate in well woman clinics
  • To ensure compliance with the practice chaperone policy
  • To prioritise health issues and intervene appropriately
  • To support the team in dealing with clinical emergencies
  • To implement vaccination programmes for adults and children and travel
  • To liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.)
  • To delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual)
  • To support the clinical team with all safeguarding matters, in accordance with local and national policies
