Nurse Practitioner – Youth Wellness Hub (Part-Time, Permanent)

Location: Scarborough, ON (Hybrid work model)

Hours: 21 hours per week

Compensation: $48.35–$68.48/hour

Employment Type: Permanent, Part‑Time

About the Role

We're seeking a dynamic and compassionate Nurse Practitioner to support youth aged 12–25 within an integrated health and wellness hub. This role offers a unique opportunity to provide rapid, barrier‑free access to primary care for young people navigating mental health, substance use, and wellness challenges.

Key Responsibilities
  • Deliver comprehensive episodic primary care for youth and young adults.
  • Perform full assessments, including health history and psychosocial factors.
  • Diagnose and manage acute and stable chronic conditions.
  • Prescribe medications and order diagnostic tests within NP scope.
  • Provide health education and integrate psychotherapeutic approaches as needed.
  • Collaborate with a multidisciplinary team (counsellors, peer workers, navigators).
  • Participate in program development, quality improvement, and community outreach.
  • Provide mentorship and contribute to knowledge‑sharing within the team.
Qualifications
  • Master’s degree in Nursing (Primary Health Care NP specialization).
  • Registration in good standing with the College of Nurses of Ontario (Extended Class).
  • Experience working with youth and/or in mental health, addictions, or primary care.
  • Knowledge of trauma‑informed, harm reduction, and anti‑oppressive practices.
  • Strong decision‑making, communication, and leadership skills.
  • Ability to work hybrid (on‑site and virtual) and flexible hours, including evenings.
