Medication Management Specialist (Remote)

In this role, you will provide clinical input on side effects and medication safety in collaboration with prescribers. Working closely with service managers, pharmacy professionals, clinical staff, and patients, you will ensure high‑quality, patient‑focused pharmaceutical care.

Key Responsibilities
  • Provide clinical input on side effects and medication safety in collaboration with prescribers.
  • Oversee all repeat prescriptions (NHS and private), ensuring alignment with titration outcomes and clinical recommendations.
  • Support and quality‑check the implementation of NHS Electronic Prescription Service (EPS) for ADHD medications.
  • Contribute to the development and delivery of ADHD services, ensuring continuity of supply and patient support across care settings.
  • Collaborate with prescribers to support safe and effective titration processes.
  • Identify and flag medication safety concerns or inconsistencies within titration pathways.
  • Liaise with cardiac physiologists to coordinate investigations (e.g., ECG) prior to medication initiation when required.
  • Participate in policy and procedure implementation within the pharmacy team.
  • Regularly review medication use, participate in multidisciplinary meetings, and ensure appropriate monitoring (e.g., blood tests, documentation of doses).
  • Manage and monitor patient referral trackers for external ADHD diagnostic reports, ensuring compliance with national clinical standards.
  • Respond to basic medicine information queries.
  • Oversee administrative duties related to diagnostic assessments and medication suitability documentation.
  • Maintain accurate records of prescribing decisions, communications, and relevant clinical data.
  • Provide liaison and advice to GPs regarding medication optimisation.
Essential Experience & Qualifications
  • Pharmacist registered with the General Pharmaceutical Council (GPhC).
  • Degree in Pharmacy, plus one-year preregistration training and final examination.
  • Independent Prescriber (minimum 1 year).
  • ADHD Prescribing (minimum 1 year).
  • Experience using CRM tools.
  • Strong understanding of confidentiality requirements across all work areas.
Desirable
  • Postgraduate qualification in mental health therapeutics (or willingness to study for one).
  • Knowledge of mental health terminology.
  • Familiarity with patient record systems.
