A community support organization is seeking a Lead Worker to provide essential support to individuals and families in Telford and Wrekin. This role focuses on homelessness prevention and tenancy sustainment. Responsibilities include conducting assessments, developing personalized support plans, and collaborating with multiple agencies. As a Lead Worker, you will provide early, targeted, and flexible support to individuals and families across Telford and Wrekin. You will work with young people aged 16-54 and older individuals aged 55+, supporting them to maintain their tenancies, achieve financial independence, and improve their health and well-being. This role focuses on homelessness prevention, tenancy sustainment, and promoting independent living.
