Qualifications

• Minimum 2 years of experience in Human Services or medical related field.

Responsibilities

• Coordinate behavioral health care with team members.
• Screen and assess patients for mental health disorders.
• Provide patient education and monitor treatment adherence.
• Facilitate referrals to mental health treatment programs.
• Manage a registry of patients in the Integrated Care track.
