A healthcare provider in the United Kingdom is seeking someone to work in a clinical capacity providing mental health interventions. The role involves supporting clients at risk of homelessness, working collaboratively with various agencies to help remove barriers to healthcare and stable accommodation. Candidates should have experience in mental health assessments and a strong understanding of homelessness challenges. This is a 12-month fixed-term contract.
To work in a clinical capacity providing mental health interventions - including assessments and treatment to clients at risk of experiencing homelessness or with a history of experiencing homelessness, as identified by Test Valley Borough Council's Housing Options Team, or Hampshire and IOW NHS Healthcare NHS Foundation Trust's community mental health or the inpatient flow teams outside of established pathways.
To contribute to breaking down the barriers to accessing and sustaining health care and suitable settled accommodation for the identified client group. To actively work with other agencies to provide wider support and address underlying causes of homelessness, developing referral routes between the Council and partner organisations across the wider system (including statutory and voluntary sector services).
This is a 12 month fixed term contract.
