What You’ll Be Doing

As a key member of the Hospital Discharge team, you’ll:

  • Support adults leaving hospital, ensuring a safe, well‑coordinated transition back into the community.
  • Carry out assessments to identify the right level of support needed post‑discharge.
  • Develop and review care plans that promote independence and reduce the risk of re‑admission.
  • Supervise and support a small team of social workers and an assessment officer, ensuring effective, person‑centred practice.
  • Work collaboratively with health and care partners to make sure no one falls through the gaps.
