Individuals with certain chronic conditions receiving Medicaid/Medicare benefits through a managed care organization.

Applicants should be aware the program provides services to support participants with serious chronic health conditions and social service needs. The program offers care coordination with medical and behavioral health, as well as long-term services and support through a person-centered action plan at no cost to the participant. Services include provider management, appointment assistance and connecting with community resources.

Services and Focus

Services to low-income individuals and families which include Head Start, services to victims of domestic violence and sexual assault, transportation, housing, energy assistance, weatherization, adult and long term care, senior nutrition, services for run-away and homeless youth, and employment and training services.
