Service: Urgent Community Response (UCR) / Admission Avoidance & Discharge Service

Contract Type: Locum

Hours: 0.4 WTE (Part-time)

Start Date: ASAP

Rate: £30

Role Overview

We are currently seeking an experienced Band 6 Physiotherapist to support the Urgent Community Response (UCR) and Admission Avoidance & Discharge Service within East London.

This role is based at the Royal London Hospital and across the community in Tower Hamlets, working within a multidisciplinary team to support early discharge, rapid response, and prevention of unnecessary hospital admissions using a Discharge to Assess model.

The postholder will rotate across:

  • Rapid Response (including Falls Pick-Up Service)
  • Intermediate Care
  • Admission Avoidance within Emergency Department
Responsibilities
  • Deliver specialist physiotherapy assessments and interventions across acute and community settings
  • Support admission avoidance and safe, timely discharge from ED and short stay units
  • Manage a caseload of complex patients with physical, cognitive and psychosocial needs
  • Work autonomously while contributing to MDT decision-making
  • Arrange and install equipment at short notice to facilitate safe discharge
  • Provide rapid community follow-up post-ED discharge
  • Contribute to service development, audit and quality improvement initiatives
Essential Requirements
  • Degree or Diploma in Physiotherapy
  • HCPC registration
  • Minimum 18 months post-qualification experience
  • Experience in acute, community or intermediate care settings
  • Strong clinical reasoning and autonomous practice skills
  • Ability to work flexibly, including late shifts and weekends if required
Desirable
  • Experience within UCR / ICT / Admission Avoidance services
  • Experience with equipment provision and discharge planning
  • CSP membership
