Responsibilities

• Provides high-quality, patient-centered, clinical care.
• Performs initial and ongoing assessment of the patient.
• Communicates and documents assessment of patient with the interdisciplinary team.
• Develops and documents individual plan of care.
• Revises care plan based on ongoing evaluations.
