Main duties of the job

To undertake duties in various aspects of Pharmacy; to provide Clinical Pharmacy Services; to optimise and ensure the safe use of medicines in the Trust; to develop personal skills, experience, professional judgement, and knowledge in all aspects of Pharmacy and the Hospital Service. There will be opportunities to rotate through areas including medicine, surgery, cancer services, paediatrics, critical care, medicines information, dispensary and many more.

Following successful completion of the NGH probationary period you will have the opportunity to be enrolled onto the Band 6‑7 Development Programme. Successful achievement of the development programme will lead to progression to a Band 7 position.

We have a very strong team ethos with good support from your peers and the senior pharmacists and technicians who understand you are learning and maybe working in a new environment. You will work closely with our team of clinical pharmacists and medicines management technicians who support our pharmacists on the wards and allow you to focus on the clinical part of the job. We are in the process of implementing electronic prescribing and medicines administration on all our inpatient wards. We work closely with the doctors and nurses on ward and board rounds and within multidisciplinary teams.

Detailed job description and main responsibilities

To use your pharmaceutical expertise for the benefit of patients and the organisation. This will include the following tasks:

Using your own pharmaceutical knowledge to positively impact the use and stewardship of medicines by providing information, advice and guidance.

To maintain your knowledge of developments in pharmacy practice as required by The General Pharmaceutical Council.

To participate in the Clinical/Advisory Pharmacy Service on a Band 6 rotation through Dispensary Services and Clinical Services (Medicine, Surgery, Critical Care, Oncology, Paediatrics, Medicines Information, and other specialities).

To participate in the emergency duty service.

To record your clinical activity and to monitor the outcome of your advice in line with departmental requirements.

To fulfil the professional requirements of the Code of Ethics and Standards of Good Professional Practice and Responsible Pharmacist.

To maintain a knowledge of developments in pharmacy practice and clinical pharmacy.

To undertake organisational responsibilities appropriate to your grade for operational efficiency and for personal experience and development. This will include the following tasks:

  • To supervise, check and, if necessary, undertake the supply of medicines. Technician checking is in place; Pharmacists make initial validations and have a professional responsibility to decide on the need for further involvement in the supply process.
  • To undertake reconciliation of medicines on admission and discharge.
  • To assist in the supervision and training of pharmacy staff, including Pre‑registration Pharmacy Graduates, Vacation Students, technical staff, healthcare staff and patients.
  • To help maintain an accurate computerised information and stock control system.
  • To ensure that legal requirements and policies of the Pharmacy Service and the Trust are followed.
  • To assist in the implementation of county‑wide prescribing strategies, cost improvement initiatives, CQUINs and QIPPs.
  • To assist in the Formulary Management process.
  • Participate in the Trust and departmental Audit Programme.
  • Participate in the Medicines Information Service.
  • Assist in the validation of High Cost Medicines.
  • To contribute to clinical guidelines, protocols and procedure development.
  • Attend Clinical Pharmacy meetings and educational/training schemes relevant to post.

When offered, to undertake the Clinical Diploma in Clinical Pharmacy to enhance the advice and information given to patients and healthcare staff to improve patient outcomes. This will include the following tasks:

  • To maintain and document satisfactory performance and progress.
  • To put into practice the knowledge acquired through study.
  • To educate and train pharmacy and other healthcare staff.
  • To undertake audit, quality or service improvement and change management.

Other Duties. This will include the following tasks:

  • To undertake/participate in risk assessments.
  • To show skills in prioritising conflicting tasks and escalating issues in an appropriate and timely manner that meets the Trust objectives to improve patient safety, patient experience, and clinical effectiveness.
  • To undertake duties elsewhere in the locality if required; this may involve the provision of clinical pharmacy duties in other locations covered by the service.
  • Undertake yearly appraisal with line manager and agree personal development plan and objectives.
