Qualifications

  • Five years’ experience in healthcare, social work, or related position.
  • Valid licensure with New York State Department of Education in Social Work.
  • Valid NYS Driver’s License.

Responsibilities

  • Provides direction in performance standards and expectations for staff.
  • Develops quality indicators to measure performance and outcomes.
  • Conducts documentation audits.
  • Coordinates and monitors all in-reach and outreach services.
  • Supervises social work staff.
  • Utilizes performance management guidelines to counsel staff.
  • Implements disciplinary processes as necessary.
  • Selects appropriate candidates for positions.
  • Provides for orientation of new staff.
  • Delivers ongoing education and training.
  • Provides social work consultation and program service delivery education.
  • Monitors internal and external referrals.
  • Conducts and responds to SafeConnect incidents.
  • Represents social work services in hospital initiatives.
  • Leads Peer Interview team.
  • Supervises lethality assessments conducted by social workers.
