A local healthcare provider in Wakefield is looking for an enthusiastic Social Worker to join its Transition Team. The ideal candidate will work closely with young people aged 16–25 transitioning from Children’s services to Adult Social Care. Responsibilities include conducting assessments, fostering independence, and managing a complex caseload. Applicants must hold a Social Work degree, be registered with Social Work England, and possess a full driving license. Flexible working and ongoing professional development are offered.
It is an exciting time to join Adult Integrated Care, where we use a strengths‑based approach to promote independence and meet the health and wellbeing needs of vulnerable young people and their carers. We are seeking an enthusiastic and hardworking Social Worker to join our Transition Team, who cover the Wakefield District.
You will work closely with key partners in Children’s Services to carry out person‑centred assessments with young people aged 16–25 who are transitioning from Children’s services into Adult Social Care.
Using a strengths‑based approach to assessment and support planning, you will:
The Transition Team consists of 4.5 FTE Social Workers. You will be joining a motivated, supportive, and forward‑thinking team where supervision, development, and collaboration are priorities.
As a Social Worker, you will:
A full driving licence is essential, as the role covers the whole Wakefield District and some young people may be placed out of area.
