Community Practitioner

The Northumberland Recovery Partnership has an opportunity for a Band 6 Community Practitioner to work within the team.

The role involves working with adults affected by substance and alcohol misuse, providing safe and effective interventions, harm reduction advice, and psychosocial interventions to support service users in considering and achieving recovery.

The team consists of a well‑established mix of professionals who work collaboratively to deliver individualised care. The role involves individualised clinical assessment, care planning, formulation, and collaborative work across the multi‑agency team.

The applicant is expected to demonstrate leadership within the team, providing supervision to junior staff and supporting students.

The applicant will cover areas within the Northumberland Recovery Partnership's geographical area, including Blyth and Ashington.

Main duties of the job
  • Responsible for reviewing and managing casework of service users with substantial and complex needs.
  • Provide specialist clinical advice to service users/carers, staff, statutory agencies, 3rd sector partner agencies and the public.
  • In collaboration with service users and carers, develop recovery‑ and wellbeing‑focused care plans, ensuring regular reviews to monitor progress and adapt as necessary.
  • Consider carer needs and involve carer and family in care planning where appropriate.
  • Deliver evidence‑based clinical and psychologically based interventions.
  • Work as a member of the multi‑disciplinary team to ensure high‑quality individualised care that promotes recovery, wellbeing, and maximises independence.
  • Coordinate treatment planning and interventions, reviews and discharge planning.
  • Provide clinical supervision to staff as delegated by Pathway Clinical Lead and Pathway Manager.
  • Provide compassionate care based on empathy, kindness, respect and dignity.
  • Incorporate a client‑centred philosophy into nursing practice and collaborative care.

Person Specification
Education and Qualification
  • Professional qualification and current professional registration as a First Level Registered Nurse (RNMH, RMN, RNLD) or equivalent as an AHP, Social Worker or Clinical Psychologist.
  • Evidence of post‑registration training in clinical/psychological interventions/skills relevant to the post.
  • Proven post‑registration clinical experience relevant to the post.
  • Training in CPA (Care Programme Approach), Safeguarding, MAPPA (Multi‑Agency Panel for the Protection of Vulnerable Adults) and Child Protection.
  • Up‑to‑date knowledge/experience of relevant legislation.
Skills and Competencies
  • Proven ability to carry out and provide comprehensive written assessments of individuals with complex mental health needs.
  • Risk assessment and risk management / safety planning skills.
  • Demonstrated understanding of effective & appropriate treatment options including a range of evidence‑based clinical approaches as identified in N.I.C.E. Guidance.
  • Effective written and verbal communication skills.
Knowledge and Experience
  • Awareness and understanding of professional accountability, NMC standards and other local and national initiatives.
  • 12 to 24 month band 5 qualified nurse experience dependent upon service need.
