A Family Health Team in Greater London is looking for a Nurse Practitioner to deliver person-centred primary care across the lifespan. This role involves collaboration with interdisciplinary teams, focusing on health promotion and disease prevention. Qualified candidates must have a Master of Nursing and a strong background in primary care. The position offers an hourly rate between $56.63 - $69.06 based on experience. Applicants should be prepared for a criminal background check and medical screening.
The Nurse Practitioner (MRP) will work within an integrated care model, collaborating with physicians and other interdisciplinary team members to deliver person-centred care. The Nurse Practitioner will deliver comprehensive primary care across the lifespan, focusing on health promotion, disease prevention and patient education. The Nurse Practitioner will work to their full scope of practice and serve as the Most Responsible Provider (MRP) to an individual panel of patients as well as supporting the team in the provision of same day appointments. The Nurse Practitioner will participate in the development, implementation, monitoring and evaluation of programs and quality improvement initiatives for the Four Counties Family Health Team.
Hourly Rate: $56.63 - $69.06 (based on years of experience).
