Job description

Our Reablement Support Officers work with a number of people in the community each day. You’ll travel to the people’s homes to support them with regaining skills and confidence to enable them to become more independent, actively promoting their choice and dignity.

It’s an incredibly rewarding role; every day and every visit are different. You’ll work with autonomy and independence but within a close‑knitted team and will receive regular support from the Reablement Team Manager. You’ll also collaborate closely with other health partners such as the intermediate care team.

Shift pattern

Cycle: 4 days on, followed by 4 days off. Shift breakdown per 4‑day block: 2 Morning shifts (07:00–16:00), 2 Evening shifts (13:30–22:30). Total weekly hours: 31.5.

Pay: £14.88–£15.68 per hour depending on experience. Rate of pay also includes 45p per mile.

What will you be doing?
  • Visit multiple individuals at home, independently or with a colleague, to monitor progress and support day‑to‑day activities (personal care, meal preparation, mobility, community access).
  • Case‑manage reablement journeys from assessment to discharge.
  • Plan and support residents in achieving their reablement goals.
  • Signpost to other agencies (Assistive Technology, Age UK, Northamptonshire Carers, etc.).
About you
  • Confident working independently and able to handle unexpected situations.
  • Previous experience within a health or social care setting.
  • NVQ level 2 in Health & Social Care or equivalent (or willingness to work towards it).
  • Full driving licence and access to own vehicle; will travel countywide within Northamptonshire.
  • Enhanced DBS certificate (subject to an enhanced DBS certificate at cost).
