REABLEMENT CARE WORKER – JOIN OUR TEAM IN CO. DURHAM

Salary: £13.30 Location: Across County Durham

Shifts: 7am–3pm / 3pm–11pm

Work Pattern:

• Week 1: Monday–Sunday (with two weekdays off)

What You’ll Do

As a Reablement Care Worker (also suitable for those with experience as a Care Assistant, Support Worker, Home Care Worker, HCA) you will:

• Visit individuals in their homes to support safe, independent living
• Assist with personal care, daily living tasks, and light household duties
• Support with mobility, routines, and reablement goals
• Accompany individuals to local amenities or appointments
• Work closely with the Hospital to Home team to support smooth discharges
• Follow personalised care plans and record progress
