Responsibilities

  • Support the Clinical and Operational leads to manage and lead the Discharge Lounge.
  • Co-ordinate the unit to maintain smooth flow and a positive patient journey.
  • Identify transfer and discharge candidates, complete handovers and ensure safe discharges.
  • Assist with the assessment of clients’ care needs, develop, implement and evaluate care plans.
  • Act as a skilled member of the multi-disciplinary team, promoting inter-departmental relationships.
  • Provide clinical advice on nursing issues and apply evidence-based practice.
  • Facilitate sound discharge planning and direct patient care as required.
  • Ensure high standards of care in line with the Trust’s policies and procedures.
  • Deliver teaching and support to clinical staff, including appraisals and personal development planning.
  • Clinically manage the discharge lounge, ensuring it remains safe and effective.
  • Assist the Discharge Lounge Coordinator and Operational Manager with resource management.
  • Maintain an adequate skill mix to meet the needs of the clinical area.
  • Proactively manage transfers and discharges to maximise safety and timeliness.
  • Promote and uphold agreed standards of practice, supporting the achievement of unit objectives.
  • Integrate new nursing procedures, research and initiatives into clinical practice.
  • Serve as a councillor and adviser to junior staff and learners on nursing matters.
  • Participate in the MDT, ensuring discharge documentation is completed promptly.
  • Provide support and oversight of the escalation ward when operational.

We are seeking a highly motivated nurse to join the Discharge Lounge Service within the Trustwide Site Operations. The role utilises a 34·5‑hour week and involves working closely with all acute adult inpatient wards, assessment units and the emergency department. The Discharge Lounge at Torbay Hospital requires experienced senior nurses who are innovative, proactive and patient‑focused.
