The organization operates 805 long-term care beds and 308 seniors’ apartment units, with an additional 224 long-term care beds opening in a new home by June 2026. In addition, Yee Hong offers four campuses of managed care, a hospice residence, and a wide range of community-based senior and caregiver support services. Through this comprehensive network, the organization serves more than 15,000 individuals annually across the Greater Toronto Area.**Duties and Responsibilities:**Collaborates with the Chief, Professional Practice and Quality and other staff to facilitate Yee Hong's continued development as a learning community. Specifically:* Foster high quality safe and excellent professional and paraprofessional care and service delivery.* Develop, implement, coordinate and evaluate profession specific evidence-based practice standards that promote the vision and strategic goals of Yee Hong, reflecting changes in the Legislation and Regulatory Colleges’ standards across all programs •* Lead and support the development, implementation, evaluation, and optimization of selected clinical programs, through education, policy and procedure development, monitoring and communication of key performance indicators.* Develop, implement, evaluate, and promote clinical policies, procedures and protocols based on evidence-based practices, professional standards and guidelines.* Participate as a member of the Clinical/Quality Team and provides on-call coverage during and after business hours on ad-hoc basis.* Lead and support clinical projects as approved by the Chief of Professional Practice & Quality. Ensures local clinical teams are engaged in activities that directly affect them.* Participate and lead quality and continuous improvement activities in keeping with Yee Hong Centre’s mission, vision and strategic directions.* Keep current on emerging trends and issues including up to date documentation of selected Provincial Regulatory body’s Standards of Practice, and liaise with Provincial regulating bodies, as required.* Design and deliver formal and informal clinical education sessions based on assessed learning needs and best practices. Promote staff's access to current, evidence-based research and theory related to resident care.Enhance other senior nursing staff's delivery of clinical education based on principles of adult learning.* Provide an active leadership role in the implementation, adoption, and sustainability of the new interRAI LTCF assessment system in Yee Hong.* Initiate and participate in the development/revision of job descriptions for nursing and allied health professionals in collaboration with Directors, Program/Team Managers, and Human Resources for workforce planning purposes and to ensure adherence to professional practice scope and standards of practice* Ensure the utilization of clinical and evidence-based knowledge of person-centred and recovery-oriented care models, information technology, professional practice standards (e.g. record keeping) and data quality in the analysis, and design of clinical information systems such as the interRAI/RAI-MDS, Point Click Care etc.* Critically appraise current practices and works collaboratively to facilitate implementation of best practice guidelines. Facilitate regular assessment of nursing staff's learning needs* Collaborate in the planning, implementation and evaluation of formal and informal education sessions to meet nursing staff's learning needs.* Facilitate internal and external educational opportunities for nursing staff to maintain competencies, expand professional knowledge and skills, and promote resident/family centered care.* Manage successful clinical placement opportunities for nugsing students.* Complete other tasks & duties as requested.**Qualifications:*** Current registration and good standing with the College of Nurses of Ontario.
