Job description

An exciting opportunity has arisen for a NMC Registered Band 5 Nurse to join the Home First Team (Discharge to Assess Service) in North Staffordshire. The team supports patients in the community to prevent hospital admissions or assist with discharge following acute hospital care, delivering high‑quality care to individuals with complex needs and promoting independence, rehabilitation and self‑care. The role involves working shifts across a 24/7, 365‑day schedule, requiring independent travel across a large geographical area and possibility of working from any of the three North Staffordshire bases.

As a Band 5 Nurse you will provide holistic assessment, planning, implementation and evaluation of care as part of the D2A process. You will work collaboratively within a multi‑disciplinary team and with Operational Leadership to ensure excellent care delivery, carrying out specialist assessments and care planning with service users and carers.

Responsibilities
  • Act as a role model and motivate team members in working towards innovative and high standards of practice.
  • Participate and support the Senior Nurse Coordinator and Clinical Lead in the development of an effective team and productive working relationships throughout the Trust.
  • Support the Senior Nurse Coordinator and Clinical Lead in the development of a positive and supportive team culture.
  • Provide public health and health promotional programmes to improve health and reduce inequalities.
  • Be responsible and accountable for service delivery to clients/patients.
  • Demonstrate at all times the Trust Values.
  • Improve health and wellbeing, including mental health.
  • Provide early supported discharge.
  • Initiate referrals to other health professional specialist services and agencies and establish and monitor good working relationships with other health and social care professionals, statutory and voluntary organisations.
  • Provide patients and relatives with information and education, ensuring they have meaningful choices that promote dignity, independence and quality of life.
  • Advise the Senior Nurse Coordinator and Clinical Lead of any issues that may affect the patient or the service.
