Are you passionate about making a real difference in people’s lives? Do you thrive on helping others overcome challenges and build brighter futures? The Nelson Trust is looking for a dynamic, compassionate Community Connector to join our Recovery & Aftercare team!

This role involves supporting clients who have completed treatment and need help to further develop their recovery capital. You will act as a bridge between structured treatment and aftercare, ensuring clients receive compassionate and collaborative services.

What You’ll Do:
  • Conduct individual assessments and develop personalised recovery plans.
  • Provide crisis intervention and assist clients in accessing necessary services such as mental health support, housing assistance, and employment resources.
  • Facilitate participation in mutual aid groups and provide information on addiction and relapse prevention.
  • Monitor progress, advocate for clients, involve family members, and support volunteer activities.
  • Report all safeguarding concerns and support the delivery of recovery coaching and peer mentoring programmes.
  • Coordinate recreational activities and community events.
What We’re Looking For:
  • Experience in substance misuse support and needs assessments
  • Lived experience of addiction & recovery is highly valued
  • Strong understanding of safeguarding and recovery capital
  • NVQ Level 3 or above (or willingness to work towards it)
  • Excellent communication, IT, and listening skills
  • Warm, approachable, and a true team player
  • Ability to build rapport and maintain professional boundaries
  • Commitment to confidentiality, equality, and safeguarding
