Job description

4Doncaster PCN is seeking an enthusiastic and motivated Clinical Pharmacist to join our expanding Primary Care Network team. The post-holder will work as part of a multi-disciplinary workforce to improve patient care and support the effective use of medicines across our network of practices.

The Clinical Pharmacist will take responsibility for areas of chronic disease management and undertake clinical medication reviews to proactively manage patients with complex polypharmacy, high-risk medicines and long-term conditions.

Main duties of the job
  • Undertake structured medication reviews and medicines optimisation for patients with long-term conditions, frailty, or high-risk prescribing.
  • Support the repeat prescribing process and medicines reconciliation post-discharge or transfer of care.
  • Provide specialist clinical advice on all aspects of medicines management to GPs, nurses, and other healthcare professionals.
  • Contribute to the achievement of PCN service requirements, QOF, and other quality improvement initiatives.
  • Support the delivery of national and local prescribing guidelines and formulary adherence.
  • Assist in auditing, reporting and implementing safe prescribing practices in line with CQC and NHS England standards.
  • Lead in the reduction of prescribing of antibiotics, over the counter medicines and high cost medicines.
  • Where qualified, act as an Independent Prescriber to manage patients directly within own scope of competence.
  • Be responsible for the clinical supervision of the Pharmacy Technicians

A requirement of this role is to complete the Primary Care Pharmacy Education pathway which is around 12 months duration.

The 4Doncaster Primary Care Network was established by four practices situated in central Doncaster:

The Burns Medical Practice

The Kingthorne Group Practice

The Mount Group Practice

The St Vincent Group Practice

The network population is approximately 59000 patients across the four practices. The practices have come together as a network in order to be able to respond in a flexible and responsive manner to changes taking place in the NHS.

The establishment of the 4Doncaster network has enabled the recruitment of additional staff, including Clinical Pharmacists, Pharmacy Technicians, Physiotherapists, Social Prescribers, Mental Health Practitioners, Care Coordinators and Health & Wellbeing Coaches.

We now want to recruit another Pharmacist to join our team.

Job responsibilities

Person Specification
Experience
  • Understanding of professional ethics and application in practice.
  • Experience of prioritising work load and meeting deadlines.
  • Previous experience working within a patient facing role.
  • Strong clinical pharmacy knowledge across therapeutic areas.
  • Ability to work both independently and as part of a team.
  • Excellent communication and organisational skills.
  • Knowledge of local and national medicines optimisation priorities.
  • Experience of working in primary care or with GP practices.
  • Familiarity with SystmOne or other GP clinical systems.
Qualifications
  • Current registration with the General Pharmaceutical Council (GPhC) as a pharmacist.
  • Evidence of continuing professional development.
  • Independent Prescriber qualification (or working towards it).
