Job Summary
This is a fixed term/secondment post until 20/03/2027 to cover maternity leave. An exciting opportunity has arisen for an experienced Occupational Therapist to join our front‑of‑house Transfer of Care and Liaison Service (TOCALS) team, at Glangwili General Hospital.
Team Overview
- Proactive Occupational Therapists and Physiotherapists working closely with medics, nursing, discharge liaison nurses, social workers, brokers, intermediate care and wider community and third‑sector teams.
- Committed to delivering the right support at the right time and place, often preventing unnecessary hospital admissions.

Responsibilities
- Manage a complex caseload, including complex manual handling and patient care risk assessment.
- Work autonomously and as part of a cross‑professional team in both managerial and clinical roles.
- Provide clinical leadership, specialist expertise and supervision to Occupational Therapy staff, Therapy Assistant Practitioners, Rehabilitation Assistants, frailty support workers and students.
- Network with acute and community colleagues to coordinate a seamless service across pathways and sectors.
- Support service development through audit, clinical effectiveness and research activities.
- Speak Welsh is desirable; English and/or Welsh speakers are equally welcome.
Qualifications & Knowledge
- Degree or Diploma in Occupational Therapy.
- Professional registration recognised by HPC.
- Evidence of advanced training or short courses up to Master’s level equivalent.
- Desirable: MSc or postgraduate diploma in a relevant area, Fieldwork Education Training, membership of BAOT, Leadership training.

Essential Experience
- Extensive post‑graduate experience.
- Managing a complex caseload.
- Delivering assessments/interventions via virtual platforms.
- Inter‑professional and multi‑agency team working.
- Direct involvement with carers and families.
- Negotiation of service development and clinical governance compliance.
- Therapeutic handling and risk assessments for severely disabled patients.
- Experience of staff development, including supervision and delegation.
Desirable Experience
- Evidence of student supervision.
- Key worker / Case Manager experience.
- People management / supervision of staff.
- Negotiation of multidisciplinary working arrangements.
- Leadership of clinical networks or practice groups.
- Leading service developments & change.
Language Skills
- Welsh speaker (Level 1) – desirable.
