SUMMARY
As a Certified Home Health Aide (HHA), you will provide care to clients in their home, following their plan of care. You make a positive difference each and every day by demonstrating care and compassion to our patients.
RESPONSIBILITIES
- Patient Care & Service - Promote and restore patients' health by completing all activities listed in the care plan; collaborate with Community Health Nurse/Therapist; perform various activities such as personal care, light housekeeping, meal preparation and errands.
- Communication - Maintain effective communication to convey patient health status, treatment plans and progress; communicate with office and supervisory staff regularly.
- Documentation - Accurately and completely document client care on activity sheet.
- Compliance - Adhere to required department and system protocols, HIPPA regulations, local, state, federal requirements.
REQUIRED QUALIFICATIONS
- HHA Certified or Ability to meet one of the following eligibility requirements for Competency:
- Currently certified CNA along with completion of written test and one day HHA Skills competency class within 4 days of hire.
- Documented home health aide or nurse aide training and competency evaluation from an out-of-state training program
- The Certified HHA who wishes to be employed after a two-year lapse in employment as a HHA must take the 1 Day Skills Competency Class. They must complete a written test and pass the one day HHA Skills competency class within 4 days of hire and before working as an HHA. If the candidate has worked as a Certified HHA within the past 24 months, and is actively certified, no class is needed.
- Nursing student who has documented evidence of successful completion of course work requiring mastery of home health aide tasks within the past 24 months-Fundamentals of Nursing and must show completion of written test and one day HHA Skills competency class within 4 days of hire.
- Veteran who was trained in the United States Military as medical technician or medic must show completion of written test and one day HHA Skills Competency Class within 4 days of hire.
PREFERRED QUALIFICATIONS
- 1 year of experience working with a frail or elderly population preferred.
- Compassionate, patient focused and dependable professional.
- Critical thinking skills, decisive judgment and the ability to work with minimal supervision.
- Exceptional documentation and communication skills.
- Ability to follow verbal and written instructions.
PHYSICAL REQUIREMENTS
M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.
PAY RANGE: $18.65 - $19.75
