As a Care Needs Assessor, known internally as a Trusted Assessor, you will be integral to maximising our customers' independence. You'll carry out initial assessments for referrals to our services, liaising with professionals to devise and deliver outcome-focused support.
Developing a comprehensive knowledge of local and community resources and services, you will continue to liaise with the customer and their circles of support to ensure all parties are kept up to date on progress during subsequent customer reviews.
