Fixed term Apprenticeship of 15 months duration, full time (37.5 hpw).

Home, a place where you belong

Have you always wanted to develop a career in Adult Social Care and to gain the skills to grow and thrive in the sector? Are you ready to be the first point of contact for those seeking support?

We’re looking for a proactive and empathetic Referral Coordinator to join our Single Point of Access team within the Durham Mental Wellbeing Alliance to help ensure individuals are placed in the right services at the right time.

Our award‑winning apprenticeship programme has been designed to get our customers and colleagues on the career ladder! Don’t be put off by the word apprentice as you can be any age to apply.

What you’ll do
  • Receive and triage referrals into the Alliance from a range of sources including health services, social care and housing providers.
  • Gather further information from agencies and individuals to support the referral process and provide high‑quality customer service and administrative support across the team.
  • Make informed decisions on suitability and the most appropriate services to meet individual needs.
  • Maintain accurate records of referral outcomes, support needs and risk assessments.
  • Liaise with internal teams and external agencies to ensure smooth referral pathways.
  • Promote our services to maximise referrals and build strong relationships with stakeholders.

The Durham Mental Wellbeing Alliance was commissioned by Durham County Council in April 2021. It brings together a group of organisations that offer different types of support with a wide variety of issues that can impact someone’s mental health and wellbeing. You’ll experience what it’s like to work as an Alliance and gain lots of skills, knowledge and training along the way.

You have
  • Ideally you have GCSEs in English and maths, but don’t worry if not, you will complete these as part of your apprenticeship learning.
  • You meet the entry requirements of the training: a fantastic attitude, a keenness to learn and build brilliant team relationships.
  • Good IT skills.
  • A genuine interest in learning and personal and professional development.
  • Good communication skills including handling sensitive information with professionalism.
  • You are a current or previous customer of Home Group or currently live in a Home Group property.
  • You will be doing a level 2 qualification over 15 months.
  • Enthusiasm to work in a hybrid role, based at least two days a week from our office at Mercury House, Durham, and ability to work from home on other days.
  • Available for 20% of your working week built in time for study and support for working towards your qualification.
  • Acceptable Enhanced DBS check (we pay for it).
What’s in it for you?

34 days leave (including bank holidays and a “me day”), and time off for volunteering too!

Health cash plan saving you from £1,140 per annum. We’ll cover your costs (and kids if you have them) towards dental, optical, prescriptions, alternative therapies, and lots more.

Over 800 high street discounts on groceries, holidays, and days out. You can also double up many discounts with the NUS Student discount card that we pay for!
