Expected pay for this position is $29.30 - $34.47/hour. Actual pay will be determined by experience, skills and internal equity. This is a salaried position.

Overview

Work one day from home after first 90 days.

The Ambulatory Complex Care Manager promotes the achievement of optimal clinical and resource outcomes by assessing, planning, implementing, coordinating and monitoring interventions and develops a comprehensive plan of care for the patients. Provides ongoing communication and collaborates with the Interdisciplinary Care Team to promote quality of care. Coordinates health care services to optimize clinical outcomes and patient satisfaction. Assesses and intervenes on complex psychosocial and behavioral health issues and supports population management and transition plans. Demonstrates necessary knowledge and skills to provide services to the assigned patient population. Works with Care Management Division leadership to align with strategic goals to meet OSF Key Results.

Qualifications

Required Qualifications:

  • Education: Bachelor’s degree in Social Work (BSW)
  • Licensure/Certifications: Social Work
  • Compliance with Illinois and/or Michigan state licensing and certification requirements or jurisdiction(s) where practicing. SocialWorker with a BSW who has not obtained 3 years of supervised clinical hours will be given an opportunity to complete this clinical practicum required to sit for the examination/apply for licensing for practice of social work.
  • All BSW mission partners must obtain applicable license within 4 years from date of hire. Mission partner must identify clinical supervisor by day 91 of hire, complete supervision within 3 years, submit state application within one month of completion. If supervision hours documented, they will be deducted from the 4‑year requirement.
  • Social Worker with an MSW who has not obtained license will be required to obtain license within 1 year from date of hire. Must start licensing application process by day 91 of hire.
  • Experience: 3 years of experience caring for patients
  • Other Skills/Knowledge: broad knowledge of chronic illness and medical and community resources; working knowledge of a clinical EMR system and Microsoft Office; flexible work schedule—weekends, holidays, on‑call rotation; ability to use independent judgement and make decisions with little supervision; substantial interpersonal skills; positive empathetic approach and effective communication skills to patients/families.
  • Preferred Qualifications: Master’s degree in Social Work (MSW); experience with chronic, complex illnesses adult and/or pediatric care management.
